Terms and Conditions:
- There is no limit to the number of referrals that can be collected during this period.
- Referred patients must explicitly inform Rosenberg Ortho about the referrer at the time of their consultation or initial contact.
- For each referred patient who starts orthodontic treatment, the referrer will receive (1) $25 Visa Gift Card and (1) entry into our raffle.
- Referred patients, upon initiating treatment, also become eligible to participate in the referral project subject to these terms and conditions.
- This offer cannot be combined with any other promotion, offer, or discount. Participants must be 18 years or older to be eligible for rewards.
- In accordance with Connecticut state requirements and payor requirements discounts shall only be offered to cash pay patients for referrals of patients that are cash pay or commercial pay.
- Gift cards and raffle entries may be provided to cash pay and commercial pay patients for referral of patients that are cash pay or commercial pay
